MidAmerica Administrative & Retirement Solutions, Inc.

The Sparta Area School District uses a third-party administrator, MidAmerica Administrative & Retirement Solutions, Inc., to manage both its Health Reimbursement Arrangement (HRA) and its Flexible Spending Account (FSA) Plans.

Employees can view plan information for both the HRA and FSA Plans through the MidAmerica website by logging in as a participant at https://www.midamerica.biz/

  • Inital log-in: Social security number
  • Initial password: Last 4 digits of social security number

For active employees with health insurance, the District funds an HRA that can be used to cover the insurance deductible and other eligible expenses. The District maintains a “high deductible” health insurance plan. There is also a HRA Plan used to manage an early retirement benefit for health-related expenses.

The District’s FSA Plan is available to active employees. Employees may elect to set money aside pre-tax to be used for eligible medical and dependent care expenses.

The District offers a voluntary Short-Term Disability Insurance Plan for permanent employees who work a minimum of 18.75 hours per week.

The District offers a fully funded Long-Term Disability Insurance Plan benefit for permanent employees who work a minimum of 18.75 hours per week.

The District offers a voluntary Life Insurance Plan for permanent employees.

Please note that your Life Insurance plan has a conversion option.  If you wish to take advantage of this option, you must make a written application for this provision within 31 days of your last date of eligibility.  If you fail to elect this provision within the given 31 day time period, you will not be able to take advantage of this provision at a later date.  If you have any questions or need further information, please contact National Insurance Services at 800-627-3660.

Employees covered under the Madison National Life Insurance policy are also offered Identity Theft Assistance. The employee and their family members are covered at no additional cost.

 Tax Sheltered Annuity (TSA) – 403(b) Plan

The Opportunity.

You have the opportunity to save for retirement by participating in the District’s 403(b) retirement plan.   If there are any questions, you may contact the Plan’s administrator, The OMNI Group at 877-544-6664.

How Can I Participate?

You can participate in the Plan with pre-tax contributions by submitting a Salary Reduction Agreement (“SRA”) online via OMNI’s website or by submitting a completed SRA form, found on the same website, to OMNI either by facsimile to (585) 672-6194 or by mail to 1099 Jay St., Bldg F, Rochester, NY, 14611.  Additionally, prior to contributing you must open an account with an investment provider participating in the Plan.  A list of the Plan’s participating investment providers may be viewed on OMNI’s website after submitting the District’s name and state.

How Much Can I Contribute Annually?

You may contribute up to $18,000 in 2015; this amount is subject to change annually. If you have at least 15 years of service with the District or you are at least 50 years old, you may be entitled to make additional contributions. For appropriate limits for your particular circumstances, please contact OMNI’s Customer Care Center at 877-544-6664.

What If I Already Have An Account?

 If you are already contributing to the Plan, and you want to change your contribution amount or investment provider, simply complete and submit a new SRA.  See directions above for on-line and paper submission options.

What If I Do Not Want To Contribute?

If you do not want to take advantage of this program, simply submit an SRA with the option “I do not wish to participate at this time” selected.  See directions above for on-line and paper submission options.

Other Post Employment Benefit

The District is the custodian of an Employee Benefit Trust Fund. The purpose of the Trust is to set aside funds to pay future benefits. These benefits include the retiree health insurance or Health Reimbursement Arrangement (HRA) benefit available to certain teacher and administrators who meet the eligibility requirements at retirement. Additionally, the Trust is used to fund other employee benefits, including contributions to the 403(b) Plan.
